Job Description

Full-Stack Engineer

Surgo Health is building the Behavioral Intelligence layer for healthcare — revealing why people behave as they do, predicting what they will do next, and helping organizations make the decisions that change outcomes.

A Public Benefit Corporation grounded in two decades of global health experience, Surgo generates foundational behavioral data that reveals the unseen why and builds the AI products that power decision-grade insights at population scale — equipping healthcare organizations, funders, and public agencies to act on what actually shapes health outcomes: people's beliefs, motivations, and barriers.

Backed by 40+ peer-reviewed publications and trusted by partners including the Gates Foundation, J&J, Takeda, Pivotal Ventures, and the Clinton Health Access Initiative, Surgo is building a future where healthcare organizations don't just understand human behavior, but anticipate it.

Role Summary:

We are looking for a Full-Stack Engineer to join the R&D Engineering team building Derin, our AI-powered interview platform. Derin runs conversational, LLM-led interviews with thousands of participants and turns them into insight that our clients and researchers use to ultimately improve patients' outcomes.

You will own features end to end across our stack: polished, fast, accessible interfaces in TypeScript and React (the client dashboard, the participant-facing survey app and our internal admin tools), and the APIs and database behind them, in TypeScript and Python. You will work in a small, senior team where what you build reaches clients within days.

What you’ll achieve:

  • Work AI-first: use AI agents (Claude Code, Cursor and others) throughout your day to design, build, debug and optimize, and help the team keep raising the bar on AI-assisted engineering
  • Build and evolve Derin's client-facing results dashboard and participant web app in Next.js and React (TypeScript), from data visualizations and filters to multilingual survey experiences
  • Design and implement the APIs behind them (NestJS, Prisma, PostgreSQL), and contribute to our Python services (FastAPI), including access control, exports and data-retention features
  • Turn product requirements and designs into well-structured, reusable components in our shared design system
  • Keep the product fast and reliable at scale: server-side rendering, caching, pagination and performance tuning on large datasets
  • Own the quality of what you ship: automated tests, code reviews and observability
  • Help run our release process (develop → stage → production), including hotfixes and dependency and security updates
  • Build security and privacy into how we handle sensitive participant data, in a SOC 2 and HIPAA-compliant environment
  • Collaborate closely with product managers, designers, researchers and backend/data engineers
  • Contribute to shaping our engineering culture as we grow

About you:

  • You have at least 3 years of professional experience building web applications with TypeScript
  • You have strong, production experience with React and Next.js (App Router, server components/SSR)
  • You have built and maintained backend APIs on a relational database, ideally PostgreSQL: in Node.js (NestJS or similar) and in Python (FastAPI or similar). TypeScript is your strongest language, and you are productive in Python too: you can read, change and ship code in our Python services
  • You have hands-on experience with Microsoft Azure (required)
  • You have built data-heavy interfaces: dashboards, charts (e.g. Recharts, D3), tables and complex filters
  • You value testing (Jest, React Testing Library, Playwright, pytest or similar), code review and CI/CD
  • Experience with a component library or design system (e.g. MUI) and with internationalization (i18n) is a plus
  • Experience with LLM-powered features (chat interfaces, streaming responses, AI-generated content) is a strong plus
  • You are AI-native: you use agentic tools such as Claude Code and Cursor every day to design architectures and diagrams, write and review code, debug and improve performance. You know how to set them up (context, rules, MCP servers, subagents) and when not to trust their output. Surgo is an AI-driven company that uses AI for almost everything we do, not only code, and our interview process asks you to show how you work with it
  • You hold a University degree in Computer Science or a related field
  • You are independent, proactive, and able to deliver features end to end with minimal supervision
  • You are fluent in English, both written and spoken, and comfortable working remotely with substantial overlap with US Eastern and UK hours

Location: This is a Türkiye-based role (Istanbul). Surgo is headquartered in Washington, DC, with team members across the US, UK, Türkiye, Brazil and India.

🎯 Who is this job for?

Level: Middle to Senior Full-Stack Engineer with at least three years of professional TypeScript experience, strong independence, and end-to-end delivery capability.

Required skills: Production expertise in React, Next.js, TypeScript, NestJS, PostgreSQL, Python/FastAPI, Microsoft Azure, data-heavy interfaces, testing, CI/CD, observability, security, and AI-assisted development; familiarity with design systems, internationalization, and LLM features is advantageous.

Suitable tasks: Building accessible dashboards and survey applications, designing APIs and database features, optimizing performance at scale, handling sensitive healthcare data, maintaining releases, and collaborating with product, design, research, and engineering teams.

💬 Potential Interview Questions

How would you structure a Next.js application using the App Router, server components, and client components?

I would keep data fetching and secure server-side logic in server components, using client components only for interactivity, browser APIs, and local state. I would define clear loading, error, and streaming boundaries and use route handlers or backend APIs for mutations.

How would you optimize a data-heavy results dashboard handling large datasets?

I would combine server-side filtering and pagination with indexed database queries, selective field loading, and caching where appropriate. On the client, I would use virtualization for large tables, memoized visualizations, debounced filters, and performance monitoring to identify bottlenecks.

How would you design a NestJS API with Prisma and PostgreSQL for participant interview data?

I would separate controllers, services, validation, authorization, and persistence concerns, using DTOs and consistent error handling. Prisma transactions, relational constraints, appropriate indexes, and carefully designed pagination would help preserve data integrity and performance.

How would you implement access control for sensitive healthcare data?

I would enforce authentication and authorization on the server for every protected resource, using role- or attribute-based permissions and tenant isolation where needed. I would also apply least privilege, audit access, avoid exposing sensitive fields unnecessarily, and validate authorization independently of the UI.

How would you contribute to a FastAPI service when TypeScript is your primary language?

I would first understand the existing project structure, dependency management, Pydantic models, async patterns, and test conventions. I would make focused changes with typed request and response models, preserve API contracts, add pytest coverage, and verify behavior through integration tests.

How would you build a multilingual participant-facing survey experience?

I would use an established i18n framework with translation keys rather than hardcoded strings, including support for pluralization, date and number formatting, and right-to-left languages if required. I would also define fallback behavior and test layouts for text expansion and different locales.

How would you handle streaming responses from an LLM in a React application?

I would expose a streaming endpoint using an appropriate protocol such as Server-Sent Events or a streaming fetch response, then incrementally update the UI while handling cancellation, retries, and partial failures. The server should validate inputs, enforce authorization and rate limits, and avoid logging sensitive prompt or response content unnecessarily.

What testing strategy would you use for this full-stack product?

I would use unit tests for business logic, React Testing Library for accessible component behavior, API and database integration tests for contracts, and Playwright for critical end-to-end flows. Tests should cover permissions, multilingual behavior, error states, data exports, and privacy-sensitive workflows.

How would you deploy and operate this application on Microsoft Azure?

I would use separate development, staging, and production environments with infrastructure and configuration managed consistently, secrets stored in a secure service such as Key Vault, and CI/CD gates for tests, migrations, and security checks. Application Insights or equivalent observability would provide logs, metrics, traces, alerts, and deployment health signals.

How would you design data retention and deletion features in a SOC 2 and HIPAA-oriented environment?

I would define retention policies by data type and legal or product requirements, then implement verified deletion or anonymization workflows across primary databases, exports, caches, logs, and backups where applicable. Every operation should be authorized, auditable, idempotent, and tested to ensure deleted participant data cannot be unintentionally restored or exposed.
